Explainer Video on Patient Readiness for Hospital Discharge

How can patients better assess whether they are ready to leave the hospital? This is the central question addressed by the MONITORentlassB research project at Baden-Württemberg Cooperative State University (DHBW Stuttgart).

As part of the project, a questionnaire was developed to help patients assess their own readiness for hospital discharge. This involves more than their physical condition. It also considers their mental and emotional well-being, concerns or problems they may have, the support available to them at home, and their understanding of their ongoing treatment.

The explainer video helps patients understand what readiness for hospital discharge means and why their own assessment is important. It explains how to complete the questionnaire and encourages patients to answer the questions honestly, either independently or together with relatives or members of their care team.

My role

My task was to translate the complex subject matter into clear and accessible visual communication for patients.

Based on the initial technical content provided by the client, I refined the voice-over script and developed the storyboard for the approximately two-minute explainer video. The visual design was tailored to the target audience and the sensitive situation surrounding hospital discharge. A clear visual language, a calm colour palette and a friendly tone were used to provide orientation and create a sense of reassurance.

The result is an animated healthcare communication explainer video that guides patients through the topic of readiness for hospital discharge and helps them assess their own situation.
